Client Contract

Both parties agree to abide by personal and professional boundaries expected. The relationship will be strictly professional and have zero tolerance for romantic or sexual. There will be no financial lending. The sharing of information and consent will be by written release of information provided for the family of client, therapist or treatment centre. Record keeping will abide by the privacy act digitally and physically will be secure.

The responsibilities of the coach are to respectfully listen, engage, motivate, support, provide information and resources. The coach will maintain healthy boundaries, confidentiality and personal information of the client.

The responsibilities of the client are to be respectful, show up on time, be honest, do homework to the best of their ability, have integrity, and be sober and present for each session.

Both parties agree to the rate discussed of $ ______ per hourly rate with an expiration date of one year after sessions begin. Payments can be made through e transfer, cash or credit. A retainer of six hours is required. The sessions will be one hour time blocks unless agreed upon by both parties in writing. The sessions will be in person, on line or by telephone. The contact may be terminated by either party. A 24 hours notice is required for cancellation policy. Client is subject to pay for a full one hour session if notice is not provided.

Both parties are held harmless against claims. The provided services do not guarantee results. Not subject to legal action.
